The third of four professional roles courses emphasizes the nurse's role in assisting patients during care transitions, with a focus on principles of safety, quality, and care coordination across multiple settings. Intraprofessional and interprofessional collaborations, information management, and transitional care models are explored. Experiences include case management and care coordination across the lifespan via seminars, simulation, projects and/or care transition settings. Prerequisites: Full Major status in Nursing.
